One Story, One Moment …
One of our patients who was admitted to Pataskala Oaks Care Center, wished to visit her home/farm one last time.
The patient, Social Worker and Transportation Coordination team worked together to schedule the Mobile Care Unit (MCU) to provide transport along with the MCU staff to be present with patient during her home visit with caregiver and friends.
She was able to see her 4 horses, tractor and dog Peanut. She was able to sit on her porch as she did so many times before and interact with her caregiver and friends. Then she returned to the facility after an hour-long visit; tired but thankful for the opportunity to see the farm she loved so much. She was grateful to share her experience with the facility staff upon her return.
During a follow-up call from the Social Worker, the caregiver had the opportunity to speak about the visit. “Oh, it was wonderful, just wonderful! It was really perfect.” She continued with, “it couldn’t have gone any better and that she was very happy the visit went well. It was greatly appreciated by all of us.”
The Social Worker assured the caregiver that it was an honor and a privilege for our team to be able to provide joy to the patient.
